Registering with the surgery

To register with Silverdale Medical Practice:

Register online here: Register with a GP surgery (gp-registration.nhs.uk) this will automatically register you with the practice as long as you are within the catchment area.

You can also come into the practice and collect the registration form at the front of the building, facing the pharmacy and fill them in.

You must book a new patient health check as this helps us to help you better if we have your medical details.

Named accountable GP

All our patients have an allocated named GP, which is your registered GP. If you wish to know who this is, please ask at reception.

Please note that you can still choose to see the GP of your choice.

Silverdale is a Safe Surgery

Everyone in the UK has the right to free care from a GP. Silverdale Medical Practice is proud to be a Safe Surgery for everyone in our practice area. If you don’t have ID or proof of address and you live in our practice area, you can still register with us. We won’t ask for immigration documents.
